I think Duke Thomas has no nosocomephobia (phobia of hospitals) because of that happened with his parents.
Maybe he comes across someone who needs immediate medical attention but he's paralyzed when he sees the ambulance and can't accompany the victim who very much wanted him to come with...
That'll be sad and has fic potential for a story where the Batfamily (especially Cass, Bruce and Damian) can help him overcome this stuff with therapy and a good support system (ft.his boyfriend Virgil Hawkins, his friends from his We Are Robin days and his cousin Jay of course)
