did you ignore miranda's or mordin's loyalty missions or not do them right away? it could have also happened because you did another mission before the suicide mission

No, I lost Mirandas loyalty because of the dispute with Jack and I kept going back to her after getting Paragon but I never had the option to get her loyalty back.

And as for Mordin, I guess wrong place, wrong time.

I should have sent him for the Escort. But I had his loyalty.
