someone who's never used a diaper before (maybe they've worn them before, maybe not. either way they've never actually used one before) needing to go so so badly but their body just won't let them relax enough to let go so they're squirming and whining
and then someone else reassures them that they're safe and gently coaxes them into letting go while holding them. so they bury their face in the crook of the other's neck, whining as their body finally releases and they wet their diaper for the first time

you're allowed to pee at the top of every hour, which isn't that bad, really. in theory, it should give you plenty of opportunities to make it to the toilet without having to hold it
unfortunately, you just keep finding yourself fixating on whatever you're doing. you miss one pee break, spending the time playing a video game instead. oh well, it's not that bad, it's just one, you can try again in an hour. and then before you know it, an hour passes and you've yet again missed your chance. you're squirming now, an attempt to distract from the growing pressure, and it almost helps. you resolve to make sure you get to the toilet at the next opportunity
and then you wind up fighting a tough boss, one you were sure you had enough time to beat before you went to the toilet- but then by the time you're done, you check the time and its already quarter past. how did you misjudge the time that badly?
you find yourself unable to play your game after that, you can barely sit still enough to rest your hands on the keyboard, and your concentration just isn't with the game anymore, every ounce of energy in your body is directed at keeping your piss contained. so you find a tv show to watch, something to pass the time while you wait for your next chance- just one more episode, just one more, you need to see how they resolve this plotline. you shift in your seat, both from excitement and desperation- and of course, you miss your chance again. you hardly even notice at this point, rocking your hips in your chair as you try your best to prevent yourself from having an accident
eventually, even watching tv takes too much effort to focus on, but you can't bring yourself to turn your eyes from the screen. how long has it been now? how many chances to relieve yourself have you missed? it doesn't matter. a startling clarity strikes your thoughts as your focus wanes for just a moment, but that moment is all it takes for a trickle of piss to spurt into your pants. you manage to cut it off after a second, but then your muscles relax once more against your will, too tempted by the momentary release and worn out from your hours and hours of holding, and before you know it you're gushing piss into your pants, entirely helpless to stop yourself, all you can do is throw your head back and moan with relief. it feels too good to finally let go, you can't even bring yourself to care that you're soaking your clothes and the chair

padding?
character likes wearing padding!! it's comfy! but... they've never actually used it before. either it's never crossed their mind, or they've been too shy to actually allow themself to do that. but then while they're padded, they realise they're in a situation where they can't reach the toilet, and have to decide between trying to hold it (despite how difficult it's getting, how insistently they can feel their bladder throbbing) or just letting go
they have to wear diapers through the night just in case they wet the bed, and it's fine, they don't have any issues with that. but then they have a friend staying over, and there isn't anywhere else for them to sleep- and they can't just let their friend sleep on the floor that'd be rude, and their friend won't let them sleep on the floor either, so they have to awkwardly explain the situation (bonus things to consider: how does their friend react? are they supportive, do they not care? and, do they end up wetting themself during the night? do they have to sneak out of bed to get changed? does their friend wake up and offer to help?)
it's their first time wearing a diaper, and they really like it! but they get too caught up in how comfy they are in it that they forget to go to the toilet until it's too late, and they wet themself in it when they maybe weren't prepared to go that far yet
going shopping for diapers/pullups with cute patterns! maybe they bring a friend with them who's also interested and they have a fun time together looking through everything!
also for the sake of interpreting the word 'padding' in a non diaper way. thinking about someone getting all their warm thick clothes on because its so cold outside so they can go out and maybe have fun in the snow if there is any, only to realise after a bit that they really need to go. but it's fine! the clothes they're wearing are really thick, so surely that'll be enough to absorb it even if they end up having a tiny accident. unfortunately, they Do Not have a tiny accident. they needed to go far more than they realised and by the time they realise there's going to be an extremely obvious wet patch on their clothes it's far too late for them to stop

staying over with a friend and they loan them some sweatpants to wear to bed. they wake up in the middle of the night absolutely desperate for the toilet, and they refuse to let themself piss their friends pants so they try to stand up to get to the toilet, despite how much effort it takes and the fact they can practically feel the piss sloshing in their bladder with the movement. do they make it in time, or do they not even manage to fully stand up before their hurried movements cause them to have an accident? or maybe they manage to stand up, but realise they can't make it to the toilet, so instead they opt to just remove the sweatpants and piss themself without any clothes covering them. at least then they won't ruin their friend's clothes
something about. very obvious piss stains in light grey sweatpants. even if they just leak a tiny bit it's obvious
