Posttraumatic eye deformity surgery This middle-aged man met with a horrific accident approximately three years ago. He suffered direct trauma to the region of his left eye. There was a blowout orbital floor fracture with herniation of orbital contents into the maxillary sinus. This left him with residual eye deformity. He has already been operated twice elsewhere, but both surgeries were failures. There was no improvement in the deformities involving the right orbit. He has been having double vision since that time. This has hampered him in the discharge of his duties as a heavy machinery operator. Feeling vexed with this, he had approached many hospitals and had been directed to our hospital because of the complexity of the corrective surgery. Dr SM Balaji examined the patient and obtained a detailed history. He explained to the patient that he had a widened canthal bay deformity. His medial canthal ligament was also lowered. There was orbital dystopia along with lateral canthal scarring from the previous surgeries. He also had restriction of eye movements. The left orbit was slightly lower than the right orbit. In addition to the above findings, he also had excessive scleral show. The treatment plan was explained in detail to the patient and he consented to surgery. Under general anesthesia, a bone graft was first harvested from the medial cortex of the ilium. This would be used for reconstruction of the orbital floor in conjunction with the mesh. A lateral canthal incision was then made to gain access into the operative field. The canthal bay was then aligned properly. A transconjunctival incision was then made to gain access to the region of the orbital floor. All entrapped tissue was released from the site of blowout fracture. A mesh was then used along with the graft harvested from the ilium to reconstruct the floor of the orbit. This resulted in reestablishment of symmetry to the patient’s eyes. Position of the medial canthal ligament was also normalized.
