In that particular scenario I was talking about before, I think Kaylee has the ship factory she wanted (as mentioned in Better Days) and that Simon returns to working in a hospital just like he missed doing. I sort of think River sticks around for a while until they're properly settles, there are constant visits though. Or, Simon in Better Days begins playing around with the idea of running a hospital ship, like the one they visit in Leaves on the Wind, so I think that's nice too.
