If your OC was a romanceable character then how would their approval be gained? What actions would increase their approval rating?
Izar wants you to ask her advice and take it. She favours straightforward and practical methods, and even though she'll suggest violence, she approves when you talk her out of it. She approves if you express appropriate fear of the Urge and Bhaal. She approves if you help her chase down clues about her past, including if you have to push through her resistance or fear.

She disapproves if you ask her advice and don't take it, or otherwise suggest you don't value her opinion. She disapproves (even though she won't outwardly complain) if you encourage her to give into violence or the Urge. She disapproves if you're dismissive of the Urge or don't seem to take it seriously, and if you won't help her follow up on clues about her past or cut off opportunities for her to talk to people who recognise her.
What would their character storyline be? What would be the consequences of the player following or not following it through?
Obviously Izar is the Dark Urge, so there's already a storyline there. Much like the other companion storylines, you can choose to nudge her towards either embracing or rejecting the Dark Urge. Sort of like Shadowheart, if you get your approval even moderately high, she'll default to resisting without you having to do much more; if you totally ignore her or piss her off, she'll default to embracing Bhaal and you have to pass a very high persuasion check when she transforms in the temple, or you have to kill her.
Obviously the Act 2 Dark Urge scene with the lover still happens, but I think what happens in Act 1 is that you wake up and see Izar in a sort of trance, and Sceleritas Fel directing her to kill Alfira. I think there's some insanely high check to potentially stop it, but it's basically impossible. Then you explain to Izar what happened, and you both decide if you're going to tell the others or conceal it.
I think you'd have the option to let her go 1v1 with Orin, but you wouldn't have to, but helping her makes the fight a little easier because Orin initially targets Izar. If you get through the fight without Izar ever going down, you don't have to fight the remaining cultists.
Orin won't fight you unless Izar is at least in the party; if she has permanently left the party or wasn't recruited, her body is in Orin's bedroom.
