In LKH, The Doctor was dying and unable to push through it, but River (Mels at the time) saw that he wasn't the monster she had been led to believe he was, so she used her... regenerative energy to help him... i think

*edit: AHA I REMEMBER NOW: The poison she used made The Doctor unable to regenerate, and he did actually die for good, but then because of what she had seen of him and talking to Amy she realized he was a good dude so she used up all her regenerative energy and regenerations themselves to bring him back to life
