This all depends on how much time Giovanni would spend around Leo trying to calm him down, but because Mikey sticks around him the most (in cannon) he is the first to really get close to Leo. If Van does the same he’d probably get closer to Van first, because twin thing. Since Mikey in canon gets close first he says Mikey’s name first.
So this can go two ways. He either says Mikey’s name first probably making Van jealous, or he says Van’s name first.
Contrary to popular belief, Van doesn't want to be around Leo. He hates seeing him like that, aggressive and wild and so different to the brother that they lost.
Van loves him, he does, but seeing Leo like that makes him sick, and stirs this insatiable anger that's brewing inside of him, longing to murder every single person or creature that's responsible. How could someone break down such a strong figure? To reduce him to something as painful as this?
Van's heart aches as he watches Mikey from afar, the only brother Leo is allowing near him right now. He's not jealous, nor is he afraid for Mikey's safety, but he hates that it's come to this - needing to coax the old Leo back from where he's disappeared deep inside, if he's even there at all.
There hasn't been much change though; Leo still hisses at them, swipes dangerously at Raph and growls at Donnie and stares at Van like he's prey.
He's close to Mikey though. Van's never been more grateful for his little brother.
He keeps watch over the two as they converse, with Mikey chatting absentmindedly and Leo curled up on the floor at his side, half attentive and half flitting his eyes about for danger. Van sees when Leo lifts his head gently and prods Mikey's arm with a tenative finger, when Mikey stops talking and leans down to Leo's level.
When Leo's mouth moves slowly, whatever noise coming out inaudible, but Mikey's face lights up like a beacon breaking through clouds.
He's jittering ever so slightly, but doesn't jump to his feet in excitement in fear of startling Leo. So he merely pans his head slowly towards Van, eyes bright and a giddy, disbelieving smile on his face.
"He said my name," comes the quiet whisper.
Van's heart swells, splits and mends itself within a matter of seconds. Tears spring to his eyes but he's smiling too.
"Oh," he says, grinning as something wet slips down his cheek. Mikey doesn't see it, as he turns back to Leo.
It's his own fault he doesn't want to get close to Leo, and why it was Mikey who manage to get through to him, and decidedly, not his twin. But Van's not jealous.
He's happy for Mikey, happy for Leo, and aching inside like his ribcage is closing in on his beating heart.
Sometimes happy moments are so incredibly sad.
