His soul merged with a "Save star" (the save points in Undertale) while performing an experiment

He's able to view all the different timelines in a universe, however he can't interact with them (such as breaking them, but he can cause a branch off from the timeline if he himself changes something within the story)

Perpetually exhausted due to his code glitch

His left eye socket is covered by a glowing Save star, and his right eyelight takes the shape of a spark

For context of this "experiement" I mentioned, Saver is an alternate timeline because his story branches off from the original Undertale. Sans was watching the player run through a Neutral route, judging them as usual. However, he kept noticing how the player would just stop at certain areas, pressing their hand on the air in front of them. He grew curious and approached the player when they stopped once again. This caused a glitch in the code since Sans is not supposed to interact with the player while they saved their game. So Sans' timeline essentially broke apart, throwing him into a void-like place while the player simply just had their game crash. Sans has been altered, having a star that replaced his left eye socket.
