SHDKSDK IT'S JUST INSTINCT, I GUESS?? also generally i try to touch as few things as possible in public bathrooms, bc i've been in far too many where they're out of soap so i can't wash my hands

They can tell bc people are probably fucking up the handle by kicking it, or leaving scuff marks on the wall. And speaking as a custodian i have seen people flush urinals with their elbows so maybe they did catch them doing it
