I'd like to believe Medic was in the second world war, otherwise he wouldn't be a "medic" or "mercenary". I'd also like to theorize, this'd be explanation of his mental instability (or egocentric personality) and what would be considered "rash" or "crazy" methods of medicine. And yes, I'm saying that medic would have been apart of the third Reich. I also estimated his age, and with math, the year in the game is 1972. He would have been just old enough to enlist as a field medic- like he is now.

((i don’t like to subscribe to the idea that medic was a nazi (pretty sure valve has said that he’s not one, if they haven’t outright stated it, they’ve at the very least done their best at not implying he is); however, i DO feel that he was probably forced into being a field medic for german soldiers, whether he liked it or not
his “mad doctor” personality is probably the result of the weird human experiments that took place in that time, but only for his interest in medicine and not because he was a nazi
i’m honestly not a ww2 buff (it’s fascinating, but i’m not super educated on the subject), but even like 25 years after ww2, i would think that people would still be concerned about the presence of nazis, and most likely would not let medic work in america as a doctor if he was one (despite him not actually having a medical license; when it actually got revoked, i don’t know)
also, medic speaks fluent english, though still with a highly apparent accent, so i would think it’s safe to assume that he has been in america for quite a while, like at least 10 years, maybe more
i don’t have this idea super laid out in a clear fashion, because it’s not really a thought i like to entertain, and i know there’s been discussion on here (tumblr) before on whether or not he was a nazi
but my idea is more that he fled, if not before the war, then after
