Yep,the second one is what he actually looks like.He didnt want anyone to start asking him about his old memories as he's trying to forget them.So he tried covering them.He can still can see but without any lens he can't realy see very well(he can wear glasses but he'll worry anyone will ask him about it so he used camoflaged lens instead).The chain scar on his neck is one of the abuse scars that wasnt able to heal fully.He also got a fake ear cover for the ripped one.
He's not eating drugs,its his daily medicine to avoid him from going insane after everything that happen to him when he was younger.
