If I have Thought Reflection and Asmodeus the Archfiend, what happens when I would draw a card? Can I order the replacement effects, both apply (i.e., Asmodeus exiles two cards), or something else?
When you would draw a card here, you have two replacement effects that both want to apply to that draw. Since neither of them take priority, you can apply either one first.
If you apply Asmodeus's effect first, then you would exile the top card of your library instead of drawing a card. Since you're no longer drawing any cards, Though Reflection no longer applies and you'll just exile the top card of your library.
If you apply Thought Reflection's effect first, then you would draw two cards. Since there's nothing waiting to modify the multiple draw, the game then parses it down into two individual draws. Asmodeus's effect will then be applied to each draw and exile a card from the top of your library twice instead.
TL;DR: You have your choice of exiling one or two cards here.
My opponent and I each control an Alms Collector and a Thought Reflection. My draw step begins. What happens?
Let’s look at the draw event and go step by step though applying the replacement effects.
“You draw a card.”
Your Thought Reflection’s effect is applied.
“You draw two cards.”
Your opponent’s Alms Collector’s effect is applied.
“You draw a card and your opponent draws a card.”
Your opponent’s Thought Reflection’s effect is applied.
“You draw a card and your opponent draws two cards.”
Your Alms Collector’s effect is applied.
“You draw a card and your opponent draws a card and you draw a card.”
All replacement effects have been applied and the draw event happens.
Net result: You draw two cards and your opponent draws 1 card. The inverse is also true if your opponent would draw a card.
If I have Damia, sage stone and thought reflection in play, with 2 cards in hand, how many cards to I draw at my upkeep? Thanks so much!
Damia says to draw cards equal to the difference between 7 cards and the number of cards in your had. You have 2 cards in hand, so Damia is telling you to draw 5 cards. Thought Reflection then will double each of those draws and you’ll draw 10 cards.
My friend has Thought Reflection and Psychosis Crawler out. He casts Enter the Infinite, drawing his deck*2. For each card he draws, Psychosis Crawler triggers once. Would he lose from drawing his entire deck, or win for draining all opponent's life?
Your opponent loses before the Psychosis Crawler triggers even go on the stack.

About your tippy-top-down design challenge card: Doesn't Zur's weirding work because it says they draw "The" card and not "A" card?
Check the oracle text of Zur’s Weirding. It says just draw a card. ZW works because its replacement effect has already applied to the event. If this didn’t work this way, then Thought Reflection could never work or it would create its own infinite loop.
