Do I understand Joels decision, and why he did what he did? Yes.
Do I think Elli would have wanted him to do that? No, the 2nd game makes that clear.
Do I think the Fireflies should have given her all of the information, so that going through with the surgery would be a fully informed decision on her part (informed consent anyone)? Yes absolutely, and I think the Fireflies were wrong for not doing that.
Do I think Ellie would have still chosen to go though with it, knowing she would die? Yeah probably, girl has a martyr complex a mile wide and so much survivors guilt (and even wothout those factors, she probably would have, but that would be a different Ellie, so hard to say)
Now would Joel have accepted her decision, if he knew Ellie fully chose to die for the world? I' m honestly not sure, interesting question I maybe will explore more after rewatching the series.
Anyway the finale was amazingly done, and was, imo (and I already said this about the game) the perfect way to end this story about the human condition and what we do for love.
