Does Itama have friends bc he also gives off no friends vibes he doesn’t talk about his feelings but I could be wrong

He does! Itama has a lot of friends, actually, probably second most after Hashirama among the senju bros. A handy chart:

But if you asked Itama's friends what they know about them, most of them would suddenly realize that they know like, his name/appearance/etc basics, and... a favorite food, maybe...? Or at least he eats it a lot so it could be his favorite?  Very little.

Itama is very good at getting people to share a lot with him while sharing basically nothing meaningful in return. It's usually not deliberate (though he is aware of it); he's just very interested in people (so he asks about them and is a good listener, so people tend to share a lot) while also being very private (doesn't share much of any significance about himself). So he knows a lot more about his friends than they know about him.

It's okay though! Just because he could take almost anyone he knows apart with a sentence doesn't mean he will. He's much more likely to subtly nudge people toward better life choices and mental health than anything.

(Which is, again, usually not conscious manipulation: Itama Gets people very intuitively, and he knows what they need to hear to react a given way without really thinking about it. It's obvious to him, and he doesn't really understand why not everyone else sees it. Similarly to how Tobirama will just Understand advanced math or how to make a seal that does whatever he wants, and it's easy and obvious for him, and he doesn't really understand how these things could not make sense to other people.)

As far as close friends that Itama is very open with, that's mostly his brothers and eventually Kunimi.

Is Tobirama aware that the Uchicha have weird feelings towards him? Also in the au how do he react to Izuna basically feeling neglected by his family? Is he aware he’s the most well adjusted or the family? how does Izuna’ twin feel about him dating him?

Well we actually have two AUs going on, so just for clarity:

Blessed Sacrifice AU: Canon, except the Uchiha believe that people with red eyes have been sacrificed by the gods on the Uchihas' behalf, making the red-eyed people weak in exchange for making the Uchiha strong/giving them the sharingan, and as a result the Uchiha are divinely obligated to protect anyone with naturally red eyes. In this AU only Madara, Izuna, Hashirama, and Tobirama are alive, because their brothers died prior to the river friendship breakup, which is when this AU diverges from canon.

Everybody Lives AU aka dt3: Canon, except all of Madara's and Hashirama's brothers are alive (which changes a few other things, such as through Madara not being clan head). The Uchiha don't care about Tobirama's eyes in this AU, so their feelings about him are purely because of his being a Senju/former enemy (ooooor just because he's a mad scientist who will insult you to your face). But not because of his eyes. (Dt3 specifically refers to Taka/Kawarama/Meguru, but since we started out focusing on them it gets used for the AU as a whole sometimes.)

We meant to just quickly figure out some personality traits for Madara's dead brothers so that he and Izuna could angst about them in Blessed Sacrifice, but then they grew too much personality and the whole Everybody Lives AU happened. We're still telling ourselves that we're only writing Blessed Sacrifice and Everybody Lives will wait at least until most of Blessed Sacrifice is written, but a few other people have expressed interest and one fic has been posted (mind the tags though, it's kinky smut). And of course there's the blog name.

On to actual answers!

Tobirama doesn't really know how to react to Izuna feeling neglected. He is not a feelings guy nor a therapist. It explains some things. (Such as: why did Izuna hang around Tobirama long enough to become friends in the first place? Why did Izuna say he liked that Tobirama ignores everyone? It's because Izuna expects people to ignore him but at least with Tobirama that just makes Izuna the same as everyone else.)

He might try to get Izuna to hang out with Hashirama (very friendly! Will definitely hug Izuna, which Izuna... probably needs...?) or Itama (best at understanding people of the brothers, would probably have been a psychologist in another life, can probably figure out exactly what Izuna's issues are and what to do about them). But Tobirama is aware that he is Not Good At Feelings, and besides Izuna seems to enjoy being around him, so probably whatever he's doing already is right, somehow? So Tobirama will mostly continue as he was.

(He might inform Izuna that Izuna has low standards.  He is correct about this.)

Tobirama might point out that there's a reasonable argument to be made that he's the most well-adjusted of the family, but he'd mostly do it to be snarky/shame his brothers into being more well adjusted themselves. (This is, of course, not very effective.) He probably doesn't actually think he's the most well-adjusted, although that's partly because he doesn't care about being well-adjusted and hasn't really thought about it much at all.

Kunimi baby darling Kunimi

Kunimi has Extremely Complicated feelings on... everything. The peace, the Senju as a whole, Tobirama in particular. The peace being based (not entirely, but largely) on Tobirama making medicine for Kunimi means that the hold the Senju have over the Uchiha is Kunimi, and Tobirama is the one doing the holding. (Also Itama, because Tobirama believes in redundancy should he die unexpectedly, so Itama knows how to make Kunimi's medicine too. But Tobirama invented it.)

And it's not that powerful, Taka isn't Madara, and if he decided that the Uchiha would be better off going back to war, he would do that and let Kunimi suffer through not having medication again. And the Uchiha have benefited a lot from the peace.

...But it still happened because the Senju were able to use Kunimi to pressure the Uchiha into making concessions.

Kunimi angsts A Lot is what I'm getting at, and Tobirama is pretty strongly associated with that. So Kunimi would really rather avoid him when possible.

At the same time though Kunimi has seen Tobirama nerd out about science and chemistry and medicine enough to conclude that Tobirama would be the most reluctant of the Senju brothers to go back to war, because war would mean that Tobirama would have less time for science in general and wouldn't be able to study Kunimi in particular at all. Which is... something...?

Very complicated feelings, as said.

But Kunimi is aware that Tobirama as a person is, while an asshole, not really bad or untrustworthy or anything (...accounting for being a shinobi). So he's glad that Izuna is happy and not terribly worried about the relationship (at least, not more than he would be worried about Izuna dating any Senju).

He could do without Izuna having been lured into the mad science, though. Kunimi did not ever need to know that his brother has dissected himself. He's glad that it was safe and that Izuna is enjoying his new hobby but why did the hobby have to be mad science of all things.  Itama is sympathetic but also inured to Tobirama’s science at this point so Kunimi is Suffering while Itama just goes ‘yeah he does that sort of thing.’
