More or less, yes.

A caretaker, spouse, family member, therapist, or doctor could make the suggestion, but only you know how much your disability affects you and only you know what you may need additional assistance with, so the final decision rests with you.

My doctor suggested that I get a service dog, but the person who made that choice to go through with it was me, not her.

I know service dogs need a doctor's note, but do they need official training? And need to pass an official test?

In the US, Service Dogs do not need a doctors note. They must be trained to do task(s) that mitigate your disability, and they do not need to pass any test or be certified in any way.

I think our FAQ should cover most of this.

If I'm flying with a service dog, and they're too big to fit under the seat, do I need to buy a ticket for the seat next to me for them?

Yes, that’s the way it works.
