In "Verse: What If...", Izumi's Mangekyou Sharingan manifested itself right before she fell into a coma. The scene which played in front of her triggered it. She was thirteen years old.

It took another eight years before she could activate her Mangekyou Sharingan again after three years of rehabilitation. From the eight years, she was five years in coma.

Izumi Verse: What If...

If the Uchiha massacre never happened and the Uchiha clan reached an agreement with Konohagakure, Izumi will still end up in a coma during a mission gone bad when she’s thirteen. She’ll be comatose for the next five years before she wakes up. Her recovery period is three years. She’ll live with her mother while she’s still under treatment. Two years after the Fourth Shinobi World War, Izumi opened up a shop in the centre of the village. It’s a small tea shop tucked away in an alley. There’s a small living space above the shop, and she’ll leave the Uchiha clan to live in the small apartment.
