I feel like Bellatrix had a number of miscarriages soon after the wedding which impacted on her mental health. After the 3rd one, Rodolphus decided he did not want to try again because seeing Bellatrix suffer was too much.

Also in my head it’s canon that Bellatrix was pregnant when she ended up in Azkaban but only found out when she miscarried.
