Hogwarts Acceptance Form

One of the projects for the FreeCodeCamp's Responsive Web Design certification was building a survey form.

As a Harry Potter fan, I reimagined the project as "What if Hogwarts embraced technology and send their potential student a QR code that links to a web form along with their acceptance letter? What would that form look like?"

Instead of students sending their owls with their notice of acceptance, this form will act as their virtual owl.
