well tbh it's not very clear, I personally see it as maybe a small scar?? Although I've seen a couple people say it was like a band aid of some sort.

I hope maybe that clears it up a bit?? All we know for sure is that Ivan really does have some sort of scarring and or wounds on his neck ::
