When should a Z pack be prescribed? I'm a scribe for a doctor who will give one to anyone with basic cold type symptoms that are present 2-3 days and who never even bothered to try Sudafed or NyQuil or anything. Sometimes he even gives a refill and says they can get it if they feel they need it. I feel like he gives these out too easily, but I also don't have a medical license so...
Azithromycin (not necessarily in the dosage form of a Zpak) treats several things on its own or in combination with other antibiotics. Most commonly we use it for community acquired pneumonia, COPD exacerbation, chlamydia, gonorrhea, pelvic inflammatory disease, atypical pneumonia, and strep throat (though not a first line treatment for strep). Less commonly in the US but still common in developing countries it’s used to treat things like typhoid, salmonella, shigella, and pertussis (whooping cough). We also use it in AIDS patients for treatment of mycobacterium infection.
All this is to say that a Zpack should NOT be prescribed for the common cold. There is certainly no reason to prescribe 2 courses of it for a cold. That is simply giving a patient a pill to take while their virus runs its course. Your feeling about this doctor handing it out too freely is likely correct. For colds (which are viral like 90% of the time), stick with symptomatic treatment only.
