Rattled insurers are pulling out of the state amid uncertainty over health reforms
Alice Little, a psychology graduate who lives amid the sand castle mountains of rural Nevada, is the kind of young person Obamacare was designed to help.
Her six-figure earnings mean she cannot get government insurance for the poor. Her status as an independent contractor means she does not get insurance from an employer. Her profession is another complication. She is a sex worker.
Ms Little, 27, works at a licensed brothel in the desert outside Carson City, the capital of the only US state where prostitution is legal. That is not Nevada’s only distinction: it has also come closer than any other state to a genuine collapse of Obamacare. Republican efforts to repeal Barack Obama’s reforms rattled health insurers so much that they refused to sell policies across huge tracts of the state for 2018. “We were terrified,” says Ms Little, founder of a group called Hookers for Healthcare.
Nevada’s problems offer a preview of where other states could be heading. “The reality was not only that insurance was unobtainable. It was going to be impossible,” Ms Little says, perched on a velvet bench in the lounge of the Moonlite Bunny Ranch, a pink wood slat brothel. “There were literally zero options for us. Ladies were talking about having to look for new jobs. Ladies were talking about having to move.”
Brian Sandoval, Nevada’s Republican governor, last week announced he had persuaded one insurer to offer coverage. But the national healthcare market remains beset by massive uncertainty — and President Donald Trump’s contradictory calls to allow Obamacare to collapse and to restart the repeal process have not helped. Nevada’s lesson is that the Republican failure to overhaul healthcare in Washington will not return things to the pre-Trump status quo. Instead, the past seven months of debate and upheaval, including a wave of White House departures culminating with Steve Bannon’s last week, have seriously destabilised the healthcare market.
Mr Trump’s supporters have not failed to notice. Conversations with some in northern Nevada suggest a new strain of anti-Washington sentiment is emerging. They still blame Congress for the healthcare debacle more than the president — but they sense the man who promised to drain the swamp is being sucked into it.
Jim Dunn, a retired Silicon Valley marketer who voted for Mr Trump and lives in Reno, the area’s biggest city, describes him as a “big disappointment”, although he says outrage over the president’s response to white supremacist violence is unwarranted. “The Republicans as a team — they swing and miss,” he says.
His wife Ann adds she is “not thrilled” with Mr Trump either, saying he has not got enough support from Congress. “He’d do better not to talk or tweet because he doesn’t communicate well,” she says. “But I still would not vote for Hillary.”
Across Nevada some 89,000 people secured health insurance through the Obamacare online marketplace. That slashed the uninsured proportion of the population from 23 per cent in 2012 to 11 per cent in 2015, partly because insurers were told that while they could choose which counties they operated in, they could no longer deny coverage to people they deemed too risky, such as sex workers or the sick.
Heather Korbulic, executive director of Nevada’s healthcare marketplace, says it had been on course for a “very successful” 2018 with five insurers — a record number — set to offer policies and compete with each other on price.
But as Republicans pursued an ill-fated effort to repeal and replace Barack Obama’s reforms, insurers got skittish because they feared any changes would mean they would not get enough premium-paying customers to cover the cost of claims.
They began to pull out of rural Nevada’s conservative, thinly populated regions, which are costly places to provide insurance (and home to legal prostitutes, who are banned from working in cities, as well as ranchers and miners).
In late June Ms Korbulic announced that 8,000 Obamacare customers in 14 counties would have no access to insurance next year. The experts called them “bare” counties. The collapse of Republican efforts a month later only added to the uncertainty. Last week a smiling Mr Sandoval said he had persuaded the insurer Centene to sell in the bare counties but acknowledged that insurers were still not getting the certainty they needed. “[Companies] just need to know what the rules are going to be, and given what is happened in Washington it’s very difficult to price a product and know what to do,” he said.
Finding a willing insurer, however, is not the end of the problem.
Centene is yet to announce premiums or the claim thresholds at which insurance will kick in. “We have more hurdles to go,” says Mike Willden, the governor’s chief of staff. “If it’s too pricey, then people may choose to go bare themselves.”
Tony Slonim, chief executive of Renown Health, a Nevada hospital group, decried how Washington — including Mr Trump — had reduced the Obamacare debate to the narrow question of repeal or not. “It’s crazy,” he said. “The consequences matter . . . We have years’ worth of work ahead of us if you repeal [Obamacare] to figure out what’s next.”
For now, Ms Little at the Bunny Ranch expresses relief that the governor has found a willing insurer. That gives her hope that sex workers will get their cervical screenings paid for next year. “I’m sure that had the founding fathers foreseen this issue,” Ms Little says, “they would have thrown it in the constitution and been like, ‘Give your people healthcare’. They gave us guns. Give us our coverage.”

Tell yourself how much you love you every day.
Wear clothes that make you feel sexy. For me it’s a variety of things, but put me in skinny jeans, a tank top, adidas and let me put on some highlighter && I’ll be ready for anything.
Work on yourself. Improve constantly. Instead of just being upset about your flaws, write them down and figure outhow to solve them.
Ways to hoe
