What a Journey
Intership was a roller coaster ride. I have mixed emotions about it. The 24-36 hours duty, coffee induced day, no lunch or dinner breaks, being relieved late, revalidas, almost falling asleep amidst a long operation, scary af consultants, the nicest residents and the meanest residents, same goes for the nurses Iâve encountered, 5-7 hours ORs (or even more), getting blood and whatever fluids all over me, multiple codes in a single night, getting to know friends who I can rely on... and so much more.
A lot of experience in a year. Iâve enjoyed some of it but definitely learned a lot. Despite getting sick for a month (thank you to the nicest residents who understood my condition, and to the consultant who asked me to prioritize my health over my rotation) I got through it all. Finally.
Was it all worth it?
By the way, be careful out there please! Donât take the pandemic lightly. Never get tired of taking simple precautions to reduce your chances of being infected or spreading it.Â
I need an anatomy refresher. So. Bad.
Not studyblr related...
I just wanna know what are your thoughts on husband who cheats on his wife?
what if they already have children?
what if the husband provides everything?
what if both can provide?
etc..
Also why do people cheat and can still act like they did nothing wrong? As if they didn't hurt any one. How do they do that?
What are you studying?
9.14.17 TOOTHACHEÂ
It is so freaking hard to focus! Just a single tooth to ruin my concentration. Hell week and my schedule wonât permit a dentist visit. Go away for now please, Iâll take care of you after the exams.
Hey, itâs been a while. Busy busy busy... Iâm currently staring at abdominal xray films. I am so amaze on how radiologists interpret films. Itâs so hard to differentiate one film from another, unless there are super distinct feature present. By super I mean you can easily see it with first glance. Iâm not new to this, but it still feels like Iâm new to it. This is art.
Iâm so exhausted.
I miss my bed.Â
I was able to slipped in 15 minutes of sleep.
I miss home.
Remind me, why am I here again?

Antimicrobial Agents  - Cell wall inhibitors
Based on mode of action âą divided into families based on chemical structure
 Modes of action Interference with:Â
cell wall synthesisÂ
protein synthesisÂ
nucleic acid synthesisÂ
plasma membrane integrityÂ
metabolic pathwayÂ
Inhibitors of Bacterial Cell Wall (peptidoglycan) SynthesisÂ
The Beta-lactam FamilyÂ
The GlycopeptidesÂ
Peptidoglycan is composed of N-acetylglucosamine (NAG) and N-acetylmuramic acid (NAM) repeat units, and amino acids. Each NAM is linked to peptide chain and the peptide chains are cross-linked.
ÎČ-lactamsÂ
Includes penicillin derivatives (penams), cephalosporins (cephems), monobactams, and carbapenems.
class of broad-spectrum antibiotics containing a ÎČ-lactam ring
Bacterial transpeptidase enzymes are responsible for catalysing cross-linking of the peptide chains
ÎČ-lactam ring bind to these transpeptidases â this inhibits cross-linking between peptide chains and prevents synthesis of stable PG
Cell wall synthesis ceases and the bacterial cells eventually die due to osmotic instability or autolysis.Â
GlycopeptidesÂ
Polypeptide agents - basic structural elements amino acidsÂ
Vancomycin:Â
complexes with peptide portion of peptidoglycanâs precursor unitsÂ
vancomycin is a large hydrophilic molecule able to form hydrogen bonds with the terminal D-alanyl-D-alanine moieties of the NAM/NAG-peptides
preventing PG transglycosylation reaction â PG precursor subunits (NAG-NAM+peptide) cannot be inserted into peptidoglycan matrix;
Vancomycin also alters bacterial-cell-membrane permeability and RNA synthesis
Uses:Â serious Gram positive infections e.g. MRSA wound infection
Adverse effects:
damage to auditory nerveÂ
hearing loss (ototoxicity)Â
âRed man/neckâ syndrome - rash on face, neck, upper torsoÂ
1/6 ă» Exam week.
Done with day 1.
Itâs so hot here today, every single day. :(
If there is an imbalance, itâs my sleep and wake relationship.
I literally miss sleeping, haha.
(forgive my notes)
Apparently Nickelodeon wants to build an attraction in Palawan, Philippines .Â
This might seem fun for some people, but for us itâs not.Â
You see, Palawan is known for its non-commercialized islands and untouched beaches. There are only a few resorts in there, and the government limits tourism population there. Here are some pictures of our beautiful islands:
Nickelodeon, however, wants to capitalize the island of Coron, Palawan. Theyâre going to build a resort and theme park there. They claim that they want to âspread environmental awarenessâ but theyâre really not. Building this resort will disrupt the marine ecosystem; thus destroying the environment there. Also, Palawan is our last ecological frontier in the Philippines. If theyâre going to continue to do this, more and more big companies will cash-in to commercialize Palawan- and I really do not want that to happen.
I know petitions wonât do much, but at least we can prove a point that Palawan should not be disrupted. Please sign this petition, so that it will not only show that us Filipinxs donât want this, but people from different countries as well. Please spread it around as well, so that people from different countries can be aware of what Nickelodeon is doing.
Ang aming kalikasan ay hindi dapat sirain. Maraming salamat po.
Gotta spread this myself. Iâm Filipina and the conservation of home is incredibly important to me. Pollution is already a huge issue throughout the countryâs cities. Many islands including Palawan are home to not just beautiful nature but some of the last of our untouched, uncolonized heritage.
wtf
NO. PLEASE NO. I LEGIT CRIED WHEN I READ THE NEWS ARTICLE. The article is in a legit news network. Itâs not fake. MY CHEST HURTS AND NOT IN A GOOD WAY. The developers say it wonât hurt the environment but artificial structures are artificial structures. PLEASE. PLEASE. NO.
I CAN ONLY HOPE THE PETITION IS GOING TO MAKE A DIFFERENCE. I SIGNED IT SO FAST.
EVEN IF YOUâRE NOT FILIPINO, PLEASE SIGNAL BOOST. PLEASE.
Receptors intro - pharmacology
Drugs act at four different levels
Molecular - immediate target for most drugs (eg propanolol binds to B-adregenic receptors)
Cellular - biochemical and other consequent effects (eg propanolol reduces Ca2+)
Tissue - function altered (eg propanolol decreases myocardial contractility)Â
System - function altered (eg propanolol reduces need for cardiac output, easing pressure on cardiovascular system)
Most drug targets are proteinsÂ
Receptors - for transmitter substances and hormones
Enzymes
Transport systems - ion channels, active transport
Substrates
Second messengersÂ
AntibodiesÂ
some drugs act on nucleic acids.
Receptors
âReceptors are the sensing elements in the system of chemical communication that coordinate the function of all the different cells in the body.â
Upon recognition of ligand (chemical signalling molecule), receptor proteins transmit the signal into a biochemical change in the target cell.
Cell surface receptors
Hydrophilic transmitters act on cell surface receptors
peptides
most neurotransmittersÂ
other small molecules
All cell surface receptors are transmembrane proteinsÂ
Extracellular domain - receptor site
transmembrane domain
intracellular domain - catalyic/coupling site, only present on certain receptors
Intracellular receptorsÂ
Hydrophobic (lipid soluble) transmitters act on intracellular receptors
steroids
thyroid hormones
vitamin D
Drug interaction with receptorsÂ
Agonist - activates receptor
Antagonist - binds to receptor without activating, thus presenting activation
Affinity - measure of how avidly a drug binds with receptor
Side effects occur when drugs bind to more than one type of receptor. Some bind irreversibly and most bind with weak intermolecular bonds. An equilibrium arises between bound and unbound drug.
(notes on types of receptor to follow - overview:)
Ligand-gated ion channels: open or close upon binding of a ligand
G-protein-coupled receptors: Transmembrane receptor protein that stimulates a GTP-binding signal transducer protein (G-protein) which in turn generates an intracellular second messenger
Nuclear receptors: Lipid soluble ligand that crosses the cell membrane and acts on an intracellular receptor
Kinase-linked receptors: Transmembrane receptor proteins with intrinsic or associated kinase activity which is allosterically regulated by a ligand that binds to the receptorâs extracellular domain
Capillary bleeding is the most common type of bleeding being that it can easily be controlled because, blood just oozes out. Venous bleeding is much darker and red and the flow of this is constant without any spurting. The arterial bleeding is the most serious type and life threatening because large amounts of blood can be lost in a short period of time when it spurts out massive blood. www.learninghumananatomy.com

how did you print on sticky notes?! I saw it and thought it was your writing lel
It was a bit tricky for me at first because my printer sucks. Here are some tutorials. (Though I made my own template).-Â http://iheartplanners.com/2015/05/08/how-to-print-on-sticky-notes/
