What a Journey

Intership was a roller coaster ride. I have mixed emotions about it. The 24-36 hours duty, coffee induced day, no lunch or dinner breaks, being relieved late, revalidas, almost falling asleep amidst a long operation, scary af consultants, the nicest residents and the meanest residents, same goes for the nurses I’ve encountered, 5-7 hours ORs (or even more), getting blood and whatever fluids all over me, multiple codes in a single night, getting to know friends who I can rely on... and so much more.

A lot of experience in a year. I’ve enjoyed some of it but definitely learned a lot. Despite getting sick for a month (thank you to the nicest residents who understood my condition, and to the consultant who asked me to prioritize my health over my rotation) I got through it all. Finally.

Was it all worth it?

By the way, be careful out there please! Don’t take the pandemic lightly. Never get tired of taking simple precautions to reduce your chances of being infected or spreading it.

It is so freaking hard to focus! Just a single tooth to ruin my concentration. Hell week and my schedule won’t permit a dentist visit. Go away for now please, I’ll take care of you after the exams.

Hey, it’s been a while. Busy busy busy... I’m currently staring at abdominal xray films. I am so amaze on how radiologists interpret films. It’s so hard to differentiate one film from another, unless there are super distinct feature present. By super I mean you can easily see it with first glance. I’m not new to this, but it still feels like I’m new to it. This is art.

I’m so exhausted.

I miss my bed. 

I was able to slipped in 15 minutes of sleep.

I miss home.

Remind me, why am I here again?

The Beta-lactam Family 

The Glycopeptides 

Peptidoglycan is composed of N-acetylglucosamine (NAG) and N-acetylmuramic acid (NAM) repeat units, and amino acids.  Each NAM is linked to peptide chain and the peptide chains are cross-linked.

β-lactams 

Includes penicillin derivatives (penams), cephalosporins (cephems), monobactams, and carbapenems.

class of broad-spectrum antibiotics containing a β-lactam ring

Bacterial transpeptidase enzymes are responsible for catalysing cross-linking of the peptide chains

β-lactam ring bind to these transpeptidases – this inhibits cross-linking between peptide chains and prevents synthesis of stable PG

Cell wall synthesis ceases and the bacterial cells eventually die due to osmotic instability or autolysis. 

Glycopeptides 

Polypeptide agents - basic structural elements amino acids 

Vancomycin: 

complexes with peptide portion of peptidoglycan’s precursor units 

vancomycin is a large hydrophilic molecule able to form hydrogen bonds with the terminal D-alanyl-D-alanine moieties of the NAM/NAG-peptides

preventing PG transglycosylation reaction – PG precursor subunits (NAG-NAM+peptide) cannot be inserted into peptidoglycan matrix;

Vancomycin also alters bacterial-cell-membrane permeability and RNA synthesis

Uses:  serious Gram positive infections e.g. MRSA wound infection

Adverse effects:

damage to auditory nerve 

hearing loss (ototoxicity) 

“Red man/neck” syndrome - rash on face, neck, upper torso

Apparently Nickelodeon wants to build an attraction in Palawan, Philippines . 

This might seem fun for some people, but for us it’s not. 

You see, Palawan is known for its non-commercialized islands and untouched beaches. There are only a few resorts in there, and the government limits tourism population there. Here are some pictures of our beautiful islands:

Nickelodeon, however, wants to capitalize the island of Coron, Palawan. They’re going to build a resort and theme park there. They claim that they want to “spread environmental awareness” but they’re really not. Building this resort will disrupt the marine ecosystem; thus destroying the environment there. Also, Palawan is our last ecological frontier in the Philippines. If they’re going to continue to do this, more and more big companies will cash-in to commercialize Palawan- and I really do not want that to happen.

I know petitions won’t do much, but at least we can prove a point that Palawan should not be disrupted. Please sign this petition, so that it will not only show that us Filipinxs don’t want this, but people from different countries as well. Please spread it around as well, so that people from different countries can be aware of what Nickelodeon is doing.

Gotta spread this myself. I’m Filipina and the conservation of home is incredibly important to me. Pollution is already a huge issue throughout the country’s cities. Many islands including Palawan are home to not just beautiful nature but some of the last of our untouched, uncolonized heritage.

Capillary bleeding is the most common type of bleeding being that it can easily be controlled because, blood just oozes out. Venous bleeding is much darker and red and the flow of this is constant without any spurting.
