Breaking up with your only best friend is the most dangerous thing that can happen to someone who is already battling a mental battle with oneself. The nobility involved in not reaching out to that special connection only to save them from your doom is obsolete if that person still means the world to you, even after years of no contact. All that you could hope for is that person is not too damaged because of your actions and would find solace in a more special connection.
