At those around 1900, the professional care wasn't really effective for most mentally ill people as it is today. They would likely just place them in the hospital, lock them, and let them stuffer or give them treatments that'll likely worsen their issues. Most poor people didn't get to have luxurious treatment or were lucky enough.
Kylar was diagnosed with amnesia. Suffering by constant fever and headaches, he lost most of his memory. Those electrified needles weren't helping and in fact tormented him, causing him distress.
But until Dr. PC came around, he was a bit doubtful of course, thinking they might be one of those careless staff, that opinion changed when they both had a few interactions. PC being understanding and genuinely concerned for his well-being, he grew attached to them, too attached.
PC didn't seem to be too phased by this, quite enjoying accompanying him, aswell as giving him hyponsis experiments and a lobotomy to treat his frequent nightmares. Which eventually he managed to recover a little — some normal emotional reactions for a person returned to normal, and with them vague memories returned. They did what they could do, even if PC is doing this out of profession -- it means alot to Kylar that they care about him.
Kylar would make sketches of them, sometimes giving them small gifts, and happily showing his work to PC if requested as a sign of love.
Giving other patients and staff in general dirty glares whenever they're getting too close to them. He is aware that they're just doing their job, but those awful people don't even deserve to be around their presence!
They firmly believes that Kylar suffering from amnesia, needs treatment, and only they can save him. Although sometimes life with PC causes Kylar pain, it also makes him feel happy. Perfect obedience, dependence and the desire to protect Kylar are for him a true "proof of affection" for them.
