Yeah, the Orthodox are great, such a beautiful and rich culture with that branch of Christianity! I wonât get into it too much simply because I donât want to misrepresent anything, but he goes to church regularly. :) The only thing is he refuses to grow a beard LOL, even though Sky and Dot have teased him about it.

âTwi, if youâre gonna be floated to the ED so much, you need to act like a legit ED tech,â Warriors huffed, waving a hand. âHonestly, itâs just sad that you canât start IVs.â
âEMTs donât do IVs in the field,â Twilight fired back grumpily. âYou canât expect me to use a skill I never learned! And I canât help that they keep floating me down here.â
âTsk, donât be so annoyed about it, you love being with us,â Warriors replied with a snort. âWeâre far more exciting.â
âAlso fewer kids.â Legend shuddered. âI hate pediatrics.â
âYou donât like kids?â Twilight asked before thinking about it. âYeah, you know what, that sounds about right. Theyâd be too honest about your grouchy demeanor.â
Warriors wheezed while Legend glared at his friend. âShut up! I mean I donât want to deal with them being sick and hurt. I donât have a bunch of experience with pediatrics, anyway.â
âThatâs fair,â Twilight conceded. âBut the point is, the advanced providers did the IVs, not me. Iâm just a basic EMT.â
âWell, youâre gonna learn IVs now,â Warriors clapped his hands together. âIf youâre stuck with us enough, weâre going to put you to work!â
Twilight hummed, narrowing his eyes suspiciously. âIs this even allowed?â
âDude, EMTs start IVs all the time in the ED,â Legend explained. âWars isnât wrong.â
âWell⌠fine.â Twilight finally conceded, shrugging. âWhat do I need to know?â
Warriors and Legend smiled at each other conspiringly. Five minutes later, the pair had managed to snag supplies for starting a line and a victim.
Twilight stared at Wild. âHowâd they rope you into this?â
âLegend said heâd taken me to the fancy restaurant we talked about the other day,â Wild answered cheerfully, holding his arm out. âNow hop to it already!â
âW-wait, I donât even know what Iâm doing!â Twilight waved his friend off, swatting his arm away.
âAll right, the first thing you need to know about starting an IV is what to look for,â Legend explained. âDoesnât matter what you use if you canât find a suitable vein.â
âNow, you have to understand this about veins,â Warriors continued as he showed his own arm. âThey have personalities, they have good and bad days, they have their own drama and problems.â
âVeins have drama,â Twilight repeated, deadpan, while Wild wheezed.
âOkay, but Iâve got to hear the tea from Twiâs veins,â Wild laughed. âWhatâwhat even constitutes as vein drama, Wars?â
âLots of things,â Warriors answered, grabbing Legendâs arm, causing his friend to yelp. âFor example, Legendâs constantly dehydrated, so his veins are probably on strike and in hiding.â
âHey!â
Warriors then pointed at a vein popping out of his own arm. âI, on the other hand, have veins that are clear as day because I hydrate.â
âThatâs not tea,â Wild grumbled, rolling his eyes. âWhat if Twilightâs arm vein is best friends with his wrist vein but then his arm vein is secretly best friends with his armpit vein and the wrist vein feels betrayed?â
âAlternatively, Twilight could be an illicit drug user and shoots up and his veins are destroyed as a result,â Legend cut in dully. âThatâs vein drama.â
âThe point,â Warriors said loudly, trying to redirect the conversation. âIs that you have to know what youâre looking for. Seeing a vein isnât always the answer. Feeling them is the main thing.â
âFeeling them?â Twilight asked.
Legend grabbed a tourniquet and tied it around Wildâs arm. âYeah. Good veins are bouncy. That means they have enough plumpness to them that you can use them to draw blood and give medicine. Those little spidery veins you see but canât feel are useless for that. Putting a tourniquet on allows back pressure to build up in veins, which helps plump them up.â
âDoes the hand pumping them do anything?â Wild asked as he flexed his fist.
Warriors shrugged. âEh. Sometimes. Not always, though. Your biggest help is gravity and heat. If youâre having trouble finding a vein, put the tourniquet on and let the arm hang with that gravity helps blood not flow back to the heart in that extremity. Itâs fighting an uphill battle trying to go up the arm, you know? Helps the blood pool in the vein.â
âHeat makes them pop up too,â Legend added.
âVeins love massages,â Warriors said, pointing a finger in the air like this was an important fact. âRub the spot where you expect the veins to be, friction and heat bring them up.â
Twilight laughed as he poked around Wildâs arm with his finger. âI guess they do have personalities. A little.â
âAC is your best friend,â Legend said, pointing in the crook of Wildâs arm. âBigger vein, closer to the skin, among the easier places to get access.â
âNO.â
Everyone turned to see Four approaching, hands on his hips.
âDo not put the IV in the AC,â Four argued. âEvery time the patient bends their arm it cuts off the flow of whatever infusion is going into the IV because the catheter gets kinked!â
âAccess is access,â Legend fired back, crossing his arms. âYou go for whateverâs quickest and easiest. The point is to keep the patient alive, not worry about your IV pump beeping about an occlusion.â
âCanât keep them alive if the IV pump is constantly stopping because of an occlusion!â Four snapped. âUpper arm is better!â
âYou can hardly get an upper arm vein without ultrasound help,â Warriors rolled his eyes. âWe donât have time for that.â
âI got a chest vein once,â Legend noted thoughtfully. âThat was interesting.â
âWhereâd you put the tourniquet??â Wild asked, eyes wide.
âIf you say the neck, Iâm punching you,â Twilight grumbled. âYou know Wild would believe it.â
Legend cackled.
Warriors sighed, pulling the tourniquet off Wild. âImportant to note you canât keep the tourniquet on too long. Veins get impatient too. Especially on frailer patients. Older veins donât last too long if you build up too much pressure. They just vanish. Find what you can and go for it.â
âOkay, so put on a tourniquet, look for a vein, look for a bounce because itâs about feel and not seeing it,â Twilight noted. âThen what?â
âCleanse the site and pick the appropriate sized needle,â Warriors answered. âThe size depends on situation and the patient. Traumas and strokes always get 18G at the smallest. Needles go by gauge size. The bigger the gauge, the smaller the needle. In other worlds, 18G is a bigger needle than 20G.â
âAnybody who needs to get contrast in CT has to have an 18G,â Four piped up.
âWhen in doubt, get an 18 if you can,â Legend summarized.
âOr a 16G,â Warriors smirked.
Legend gave his friend a side eye. âI remember that, you piece of shââ
Twilight blinked. âHuh?â
Warriors eyes sparkled with delight, and Legend pushed him so hard he fell off his chair, the travel nurse hastily saying, âDonât worry about it! Weâre going to talk about angle of entry now!â
Four bit the inside of his cheek, knowing better than to aggravate Legend as he himself had run into a similar issue and he didnât need the entire group to know. âUsually when youâre first inserting the needle you want to line it up with the vein, and the bevelâthe holeâshould be facing upward. The initial stick is at a forty-five degree angle. Basically as soon as youâve broken the skin and gotten through, you level out so youâre slinking into the vein instead of stabbing through it. Make sense?â
âYeah,â Twilight nodded. âThen what?â
âYouâre going to look for flash,â Legend explained as Warriors dragged himself back into his seat. âFlash is when you see blood in your needle. Itâll either show up in the catheter surrounding the needle or in the little box above the needle called the flash chamber. Depends on the needle make/model. But once you see that, it means youâre in. Thatâs when you thread the catheter off the needle and pull the needle out. Remember, the only time when a needle is in the patient is during the initial poke. After that the needle is pulled out and the catheterâbasically a strawâis all that remains.â
âNow hereâs the kicker,â Warriors said as he smoothed his hair. âSome needles are self occluding, and others are not. What that means is you have a straw in a vein pumping blood. An open straw. So if you donât attach something to it that prevents back flow, youâre going to basically just bleed your patient out over a long time. Some needles prevent back flow until you attach IV sets to it. Others donât. So to avoid making a giant mess and making your patient pass out, know which one youâre using.â
Wild groaned, leaning back in his seat. âSo are you going to stick me or not, I want to eat!â
Twilight bit his lip. âI donât know if I⌠I mean, I definitely would rather not let Wild be the first person I poke.â
âWeâll get a training arm,â Legend said easily. âOr you can poke Wars as many times as you like.â
âHey!â
Four waved the tourniquet in the air like a flag. âWhen youâre done with everythingâattached the extension set, flushed and seen blood return and all that jazzâdo not forget to remove the tourniquet.â
âWait, but we havenât talked about the extension set,â Twilight said, baffled.
Wild huffed, rising. âThatâs it, Iâm starting an IV on myself.â
âOh my gosh, Wild, fine! Weâll go eat!â Legend rolled his eyes while Twilight laughed. âHonestly, youâre such a glutton!â
Just wanted to say I adore your LU in Healthcare AU! My mom used to be a medical records clerk while I was growing up, so reading about the boys in a medical setting is almost nostalgic for me! đ She had the job while everything was still kept in huge file folders. They were kept in a separate room filled from floor to ceiling with them so you can imagine what a headache it was when you needed to find a patient file!
I donât know if youâve thought about jobs for the rest of the girls or even if that particular job is still an option with everything being digital but could one of them work in medical records or even medical coding? đ¤

I suppose medical coder would be the equivalent nowadays? Iâve put a little thought into the girls, but not much. Artemis will enter the story at some point, but sheâs a physician. Dusk is also a physician. Aurora is a paramedic and Dawn is an EMT. Tetra⌠idk sheâs in the coast guard? Something like that? Maybe she can get into medical coding to hold her over for something else? She definitely wouldnât stay in that career. Sun is studying to become a hospital administrator! Ah, maybe Sun can move to the area and do that while sheâs in school! Fable is a cardiac ICU nurse and Dot is a medical ICU nurse. Flora is studying biomedical engineering, sheâll eventually show up but in a different role.

my pre-op nurse had a huge chicken pin on her badge and introduced herself like âHi! Iâm so and so, Iâll be your nurse getting you ready for surgery, Iâm very sarcastic but also very nice.âđ I think I literally said âoh, weâre going to get along great!â (I was feeling so sick that morning and she did her best to distract me lol)
she especially made me laugh when she kept getting calls from the lab asking where all the scheduled patient specimens were and she literally told them âIâve got 8 patients to get ready for OR before 7:30.
