i work in harm reduction and mental health at a youth shelter so I am sometimes quite burnt out and will likely post about it ooc. i also care deeply about the dignity and lives of people who use drugs, unhoused people, and people having mental health crises. i am also multiply disabled, and all of this affects deeply how i write about and interact with media about the medical system.

don't be racist, homophobic, transphobic, zionist, Islamophobic, antisemitic, ableist, sexual about/towards minors, or stigmatizing of mental health and addiction. don't infantalize autistic, neruodivergent, and disabled people and characters.

i am an anti-zionist jew. i don't want there to be confusion when i post about jewish themes on here ( i write alex as an ashkenazi jew! ) and i feel strongly it is my obligation to say it, loudly, because of the ways zionism has co-opted our culture to justify genocide. free palestine. land back.

I write alex as bisexual and if i write romance with women, by god will he respect them. in canon, he is often a misogynist character, however i am not interested in reproducing that in my writing. he will be an asshole, snappy, avoidant, bad at feelings, jerkish, but i am not here to write him doing genuinely fucked up and harmful patriarchal shit.

i don’t often write smut because it’s not super interesting to me. but we know that the doctors at seattle grace mercy death / grey sloan memorial are fucking and yeah it'll probably come up.

i am plot-centric. what this means is i love to plot deeply, intertwine lore, and create unique bonds between characters. i am willing and love to have mains and exclusives, and to work these into my portrayal. it is something so special to get to watch the way a character is shaped by the plotting and writing we do together.

alex, who had a difficult time with his mother because she was ill but won’t let his father call her crazy or blame her for his failings. alex saying “who do you think took care of your kids? I was the dad. I was a kid, but I was the dad. You did your best? your best is worthless” to his father. alex who was the eldest son, who began running interference between his parents at the age of six, who took care of his mother when his dad up and left.

canon horrors that have befallen alex karev:

- he is shot in the lower chest while at work at the hospital and has to drag himself to the elevator to get help. he is treated while the gunman is still in the hospital, with minimal supplies, no anesthesia, no pain killers.

- another surgeon accidentally injects him with epinephrine during a surgery together and he collapses. this causes his heart to stop and it needs to be restarted.

- he marries the girl he is in love with because she has cancer and he thinks she’s going to die. she does not die and shortly after, she leaves him via letter and disappears, leaving him with her $200,000 of medical debt. he is flat broke.

- he is in an ambulance that is struck by lightning, which will then not start so he is stranded in the middle of nowhere with a patient and dead equipment, during a thunderstorm.

- he is arrested for beating up a guy who he mistook for doing something really bad, but he was wrong. he needs to be bailed out.

- he pisses off his mentor and she takes him off a conjoined twins case at another hospital, and she goes instead of him. this trip leads to a plane crash and his mentor losing her leg, which he is the one to amputate during surgery. other coworkers and friends of his die.

- he lies and tells everyone he got funding to do a bunch of cutting edge surgeries on orphans flying in from across Africa but he did not get the funding. the kids are arriving imminently and he is about to be levelled with millions of dollars in invoices and bills he can’t pay when something comes through at the last minute.

- he rats out his best friend for tampering with a clinical trial to help their friend’s wife, the telling of which almost ends her career and her marriage. he doesn’t do it out of a sense of morality, he just does it to help his chances to become chief resident. he becomes a pariah in the hospital and no one likes him anymore.

- he bribes the judge presiding over his best friend’s adoption case and lies to a social worker to cover up his friend lowkey kidnapping her not-yet-officially-adopted daughter from daycare.

- he narrowly avoids being attacked by a bear.

- he threatens and blackmails an abusive and violent man while he is still recovering in hospital and while he is in fact a doctor at that hospital.

"oh, so devastating." sawyer basks in alex's laughter, right at his ear, rumbling between their chests pressed together, vibrating off his cheeks. if he was a romantic, he'd tell those damn dharma scientists to find a way to bottle up the sound and sell it. they'd make good money off it, he thinks. maybe he'll tell alex that, some day, if only to hear him laugh again. for now, he selfishly keeps it for himself, and oh, what grand sacrifice it is, to have to manually pull these sounds out of him day by day. this is how love has made him incorrigible, he'll tell you.

karev talks about the latest kid who showed up at the clinic, and sawyer hums as he listen. it's always so palpable, the care he has for these kids. it's admirable. sawyer's happy to see him doing what he loves, what he was always supposed to do. they can be happy, here. "and you cooked dinner tonight?" he clicks his tongue, starting to move them towards the kitchen, though he remains attached to alex's body, making them stumble together in tandem. "honey, y'can't be the breadmaker and the housewife. what will people think of me?" his dimpled grin turned sly now, his jest an obvious inside joke between the two of them, aimed at the absurd reality of their current routine. he reaches a hand for alex's face, cups his chin and uses his fingertips to squeeze his cheeks together and make him pout, so he can lean in and steal an obnoxiously annoying peck. and another. they continue stumbling. "what did you make? smells delicious."

this is, truly, a stupid way to be walking the few short paces from the front door to their kitchen. their arms are interwoven strangely, hands resting and holding where they please ( and alex is not complaining about that ). sawyer leads, and alex has never been graceful but he makes it work, shuffles backwards to stay close, prays sawyer doesn't run the pair of them into the sharp edge of the counter and fuck up his back. he likes that his back of all things is at the top of his list of worries, and not whatever shit was terrorizing them a few short years ago ( and a couple decades from now? he tries not to think about it too much, it makes his head hurt ).

"and you should remember how lucky you are to have me." alex can't contain his big grin at the reaction he's prompted, the playful compliments he revels in. but he rolls his eyes for good measure, puts on a little front that everything sawyer does isn't just absolutely adored by him. "oh, everybody just loves you, lafleur. you don't have anything to worry about." count him among their ranks, the people of dharmaville who took to sawyer's lie like flies to honey and gave them this house, these jobs, these people to pretend to be. and sawyer's better at this, coming up with a story and sticking to it, but alex doesn't feel like he's pretending so much anymore. he lets himself forget, sometimes for days and weeks at a time, that they aren't really from here, from now.

then sawyer goes and does some annoying shit, and alex scolds him, and he'll be damned if it isn't the best part of his whole day. "knock it off." it's hard to look mad with his face squished between sawyer's fingers, hard to sound too upset when he's got a bottle of dharma initiative white wine waiting for him on the table. "it's salmon. freshly caught, none of the freeze-dried shit from the sub."

the amount of posts I’ve seen that theorize/fantasize that alex did not leave jo for izzie and that nothing in the letters is real, that he’s actually been held captive for the past multiple years by ava ( facial reconstruction patient who he dated who did lowkey stalk him off and on before being committed to psych ) and that she sent the letters and he’d never actually leave jo. just because this is the only way to reconcile his actions with his character growth and explain the immense character assassination of him leaving all his loved ones including his wife and his best friend of over ten years via letter and not saying goodbye in person. and its like, maybe i agree!

thinking about addison & alex today. it was short lived but so juicy. thinking about how he follows her around like a dog even though he tells her he’s not interested ( he’s a liar ). thinking about addison using alex to blow off steam and distract herself from the mark and derek drama. thinking about alex being absolutely enamoured with an older attending who he genuinely respects. thinking about my beautiful wife, woman of all time, dr. addison montgomery.
