People argue Artyom doesn't deserve sympathy and he didn't feel true remorse due to his possible NPD, but he's still human and feels distressed. No one got him help and his own mother enabled him, of course he would have a collapse the moment he's confronted with his behavior. Even if his self mutilation was only to gain sympathy, it's the fact he was willing to go that far shows true distress underneath.

Maybe I'm being a bit too lenient with the guy but I really don't think he'd offend again if he received mental health services and didn't have access to triggers.

Artyom might be manipulative and obviously has tendencies to be violent but who let him continue that behavior?

The whole point is, no one intervenes until someone's mental disorders get so severe to a point they do something drastic. Then it's turned around like they've been pure evil their whole life instead of getting a fresh start with the necessary precautions.
