How do you manage "resistant hypertension"? Try nephrologist Dr. Blaise Abramovitz's SMASHMOUTH mnemonic:

S-pironolactone--the best 4th medication (after a diuretic, an ACEi/ARB, and a CCB) M-edication regimen--ideally 1 diuretic (chlorthalidone) + 2 other meds A-dherence--it's not resistant hypertension if the patient is non-compliant S-econdary etiologies--the younger the patient, the more you should think about this H-ome BP measurements--needed to diagnose both masked hypertension and white-coat hypertension M-edications that increase BP--NSAIDs, OCPs, stimulants, calcineurin inhibitors, etc. O-SA--untreated sleep apnea can increase BP 12/8mmHg during the day and 15/12mmHg at night U-nderdosing--it's typically better to maximize the dose of one medication before adding another one T-able salt--aim for a dietary sodium intake of <2gm/day H-erbal supplements--things like licorice and St. John's Wort can also increased BP#TeachingRounds, #FOAMed, #HTN, #nephrology, #pharmacology, #cardiology, #primarycare

grad school is weirdly psychologically straining. idk how to explain it. But my advice is basically. If you're considering something drastic, also consider things that are less drastic. If you're considering quitting, then also consider threatening to quit. If you're considering suicide, consider quitting. Whatever drastic thing you're consdiering because you're about to fail a class or a candidacy exam, also consider just failing and continuing anyway. You know? It's not waht you wanted, but if you're already considering something outside of the sapce of stuff you wanted, consider all the stuff less drastic than it too

But I also found a source for the article screenshotted above! And folks it’s WILD. It’s an incredibly interesting read if you happen to love hearing about niche hobby drama. It’s also just a fantastically written article!

The parts I find the most interesting are about how conservative the cat fanciers association is. This isn’t even all of the parts that talk about that.

People are so mad about this cat spefically because he has female colors. An animal who couldn’t choose how it was born. This is happening in Texas by the way. Hm. I wonder how they treat trans people over there?
