welcome beauties! after what feels like 75 years, i’m finally ready to release dr. vu’s body image clinic! this mod is inspired by the sims 2 “dr. vu’s automated cosmetic surgeon”, and i wanted to bring that idea into the sims 4 with a more realistic plastic surgery experience. with this mod, your sims can go through consultations, procedures, recovery, and finally see their results instead of everything happening instantly. i also want to give a shoutout to adeepindigo, who has a mod i’ve loved for a long time that is very similar in concept, but since it’s outdated, this is my own take on a body image and cosmetic surgery system for the sims 4. this mod is really about adding more realism, storytelling, and personality to your sims’ plastic surgery journey.
ALWAYS CAS.FULLEDIT MODE (TO BE ABLE TO EDIT)
the mod has two main components: your sims can either go straight into the plastic surgery system through the dr. vu’s body image clinic app, or they can use the mirror interaction check for flaws, which comes with a custom animation and lets your sims start from the body image side of the mod first.
or, if your sims already know what they want or just wants surgery, they can go straight into booking a before and after exam and set up a consultation with the clinic.
during the consultation, your sims will be asked about their body goals, what they want to change, and what kind of results they’re hoping for. this helps make the process feel more realistic instead of your sim just randomly getting surgery with no build-up.
don’t know if your sim is ready for surgery? they can also think about their insecurities, compare themselves to others, and have special insecurity-based interactions with other sims. this adds more storytelling for sims who are still deciding if they really want to make a change.
after the exam, your sim can revisit what they learned and go over the details again before fully committing to surgery. i wanted the process to feel like an actual journey, not just one quick interaction.
once your sim is ready, they can move forward with surgery. after the appointment, they’ll receive a gift bag filled with after-surgery care items to help them recover.

the recovery system includes multiple aftercare objects and items, including medication, water, recovery snacks, pamphlets, clinic emails, and other things your sims may need while healing. your sims can experience pain, numbness, bandages, compression garments, and recovery buffs before finally checking their results.
this system is really about adding more realism, storytelling, and personality to cosmetic surgery gameplay. i wanted it to feel like more than just clicking one button and changing your sim. it gives your sims an actual experience, from the first thought to the final result.
