Marcus lay back, breathing hard. The baby’s head had already been born. For a short moment the pressure eased, but the next contraction came quickly. He felt the wide shoulders already sitting in his anal canal, pressing against his stretched hole.
He started pushing again, groaning deeply as his anus was forced to open wider.
— Nngh… fuck… it’s not moving easy… — he said through gritted teeth.
The nurse stood beside the bed, watching closely between his legs.
Marcus pushed steadily through the contraction, his hole straining around the shoulders. When the wave passed, he panted heavily and looked at her.
— They’re not turning… I can feel it. Are you going to do something?
The nurse shook her head.
— No. As long as they’re still moving forward, even slowly, and the tissue isn’t showing signs of tearing, we’re not going to attempt any internal rotation. Manipulating the shoulders inside the rectum right now would risk damaging the sphincter and the rectal wall. We’ll keep letting you push.
Marcus didn’t have time to answer — another contraction was already building. He immediately started bearing down again, letting out a long, rough groan as his anus stretched tightly around the broad shoulders.

When the contraction eased, he collapsed back against the pillows, breathing heavily.
— It’s taking so long… — he muttered. — They’re just sitting there…
— They’re moving, just very slowly, — the nurse replied. — We’re watching the tissue. If it starts to look like it’s going to tear, we’ll step in. Until then, we let your body do the work.
Marcus nodded weakly, already feeling the next contraction building low in his belly.
