The anime doesn't explicitly explain how or why Kinko loses her hand, but this is explained better in the manga/light novel

Anime Yuki brings up that if they try to lift the lever, then the handcuffs will tighten and they'll lose their hand. We see a brief scene where Kinko is pulling on the chain.

Light Novel Yuki sees Kinko's arm go all the way through to Momono's side to get the keys directly from Momono. Yuki realises that Kinko must have lifted the lever

Manga: In the manga we see things from Kinko's perspective. We see she's unable to reach the keys so she has to lift the lever

(10/23) The anime has a different method to begin the trap in the hexagonal room puzzle than the manga and the light novel

Manga and Light Novel: There are 6 levers and at this point 5 girls alive, so one of the levers is presumed to be a 'fake' or 'dead' lever. They need to figure out through trial and error which of the levers is a dead lever. They each pull the lever at the same time, but there is no change. They keep rotating and pulling the levers with no success. They think that all attempts have failed until they realize that they've suddenly been handcuffed. The handcuffs have been shot out from the lever

Anime: There are 6 levers and at this point 5 girls alive. They keep trying to pull the levers down with no success, the levers not moving at all. The handcuffs are already hanging below the levers. The girls must wear the cuffs while pulling the levers down in order to activate the trap
