🕐 The 15-day insurance clock starts only when your hospital submits the complete discharge document package.
Miss a document? Clock hasn't started.
❌ A date typo between your discharge bill and discharge summary is now an auto-decline trigger. No warning. No query letter.
📄 32% of claim rejections in Q4 2025 were because of illegible handwritten discharge summaries. That's nearly 1 in 3. The hospitals that will suffer most are the ones still running paper-based IPD files. No timestamps. No consistency. No audit trail.
