Jessica Walter played McMillan’s first “girlfriend of the week” in the 6th Season. Far from being a grieving widow, the audience got to see what a smooth talking pick up artist McMillan was.
This was how they explained why “McMillan and Wife” was now “McMillan” and launched the “girlfriend of the week” season. Not surprisingly, it was the final season.
In Season 5, Episode 5 "The Deadly Cure", it appeared that Sally had gotten a haircut halfway through the episode. But her long hair came back in the next scene. No explanation was given. Perhaps Sally decided to wear a wig instead of hat that day?
In Season 5, Episode 7, "Point of Law", Mildred, the housekeeper, didn't have a lot of boundaries when it came to her interactions with the McMillans. Case in point, she offers them milk and cookies and when they pass she sits down to enjoy them herself. Then this happens...
In the final episode of Season 5, "Point of Law", the McMillan's celebrate their Anniversary with a pizza surprise from Mildred and Enright. At the time it was already confirmed Nancy Walker wouldn't be back the next season but it wasn't official yet that this episode would mark the last appearance of "and Wife" - Susan Saint James.
In Episode 4, "Secrets for Sale", Season 5, we learn that Sally's middle name is Virginia as McMillan uses it not once but twice! The scene above closes the episode.

In Season 5, Episode 6, "Greed", Mildred contemplates the possibilities when she stands to inherit money. Luckily, McMillan is there to keep her from making a fashion faux paus.
"Jaws" was released in theatres the year before this Season 5 Episode, "Point of Law". I guess Sally was a fan?
