Top: in Mario Kart 7, there is a cliff with a lighthouse visible from the Wuhu Town battle course that has a hole in it.
Bottom: strangely, despite the same exact cliff with the lighthouse being visible from the Wuhu Loop track, it it shaped differently here and no longer has the hole.
The hole is also shaped differently in the Mario Kart 8 Deluxe version of the track.

In Super Mario Run, the Lakitu death sprites have the Lakitus' leg holes in the shell be almost completely misaligned with their actual legs. In the neutral position, the legs only slightly overlap the holes, while whenever one of the legs is raised, it does not come out of the hole at all.
This was likely overlooked during development due to never being able to be viewed up close in-game since the Lakitu would be rapidly falling off screen during this animation.

Top: in Mario Super Sluggers, the baseballs actually have an orange outline as part of their model that is (almost) never shown in-game. It can be curiously seen in their reflection in the ice at the Peach Ice Garden field (look closely at the difference between the blue glowing actual baseball and its orange-outlined reflection in the ice).
Bottom: the outline also appears whenever the ball is behind the camera, which of course cannot be seen during regular gameplay, but can be viewed with external tools.

In the Blooper Surfing Safari room in the second mission of Ricco Harbor in Super Mario Sunshine, there is a major texture distortion on the border of the track immediately to the right of the starting line.
The screenshot shows a view of it looking back from the first corner, while the animation starts off at the starting line looking to the right and moves towards the position where Mario is standing in the screenshot. Note the texture starting out normal before slowly becoming more and more distorted.

Left: internal 1994 Nintendo reference sheets for Donkey Kong Country list filenames for various official artwork, with the name being used for Donkey Kong's solo renders being "dkfivefinger".
Right: this is a reference to the fact that the earliest iterations of these renders actually had four digits on each limb, which was changed to five shortly before the game's release. They can still be seen in pre-release materials as well as some of the early guides; presumably, the name "dkfivefinger" was to make sure the user would know that this is the model that is actually supposed to be used instead of the earlier four-fingered version.

Top: in Super Mario 64, entering Jolly Roger Bay for the first time before getting the first mission's star will show the level as being covered in a layer of fog. The fog will clear upon obtaining that star specifically; unlike every other change in the game, it will for an unknown reason not be reversible on that save file even when selecting the mission again, and require starting a new file to view.
Bottom: early collision data for the game found in development files curiously treats the fog as water, which is likely not intentional. In addition to treating the fog as water when it is visible, Mario can swim in it even after it is gone (as shown in the screenshot), since the fog object is still present and merely set to not be rendered.

In World 1-2 of Super Mario Bros. 2, it is possible to go off the bottom of the screen without dying while flying a carpet as long as the player character does not go too far below the screen. If the character uses this to get below the rock wall at the start of the level, it is possible to fly up into it, but not back down.
This will result in the character becoming stuck inside the wall, as seen with Luigi in the footage, requiring either restarting the game or using the self-destruct code (pause the game, hold Up+A+B on Player 2's controller and unpause) to continue playing.
