Signed in Blood
TW: Medical trauma, Non-consensual medical procedure / organ theft, Loss of bodily autonomy, Betrayal by trusted figures / manipulation, Chronic pain / disability aftermath, Emotional distress / anger / grief, War-related injury / combat violence, Coercion / lack of consent under duress.
Beginning of the story takes place right after the "Tunnel Fight" in the mission 'Ghost Team' (near the end of the MW2 campaign). _________________________________________
The world was a blur of sirens, shouting, and the sharp tang of antiseptic.
They’d made it out of the tunnel, barely. Soap’s blood was on everyone’s hands, slick and too warm, his pulse fading faster than anyone could count. Y/n had taken the bullet meant for him; the shot had ripped through their side, lodging deep and dirty.
Everything after that came in flashes.
Cold hands pressing down on a wound. A helicopter’s blades cutting through the night. Price yelling coordinates into the comms. Ghost sitting stone-still, breathing heavily.
Then, white lights. The hospital. _ The world came back in pieces. A ceiling light. The faint hum of a heart monitor. A voice somewhere too calm for the chaos that still echoed in Y/n’s skull.
Their chest hurt, no. Burned. Breathing felt like dragging glass through their ribs. Everything smelled of antiseptic and iron. They blinked, the world doubled, then steadied again. Someone was pressing a clipboard into their trembling hands.
"Y/n," a voice said, low and urgent. It was Price’s. "We need you to sign this. Now."
They tried to ask what for, but their throat wouldn’t work just a rasp, a cough that made their vision go white at the edges. A nurse steadied the paper. Someone else held a pen against their fingers. Ghost’s shadow lingered near the bed, silent.
The words on the paper blurred into gray lines. All Y/n could make out were fragments: Emergency procedure. Their hand shook. The room felt too bright, too far away.
"Y/n," Price pressed again, voice breaking the line between command and plea.
The pen hovering. A shallow breath scraped through their throat. They signed, their signature barely legible through the tremor in their hand.
The world sank into black.
When Y/n woke again, it was quiet... too quiet. The only thing they felt was pain, dull and heavy, buried deep in their side. Their hand reached for the bandages on their torso before someone gently pushed it away.
"The doctors did all they could to save you," Ghost said, voice clear but distant. "You made it through." _ Years passed, but the body never forgot.
The scars healed wrong, the nerves sang where they shouldn’t. Y/n learned to live around the pain. The kind that came in waves, deep and electric, like something buried under the ribs was still trying to wake up. Some mornings their hands shook too much to lace their boots; some nights they fell asleep with a fever, whispering that it was just another infection, just another bad spell.
Soap healed beautifully. Stronger than ever, faster, louder. The kind of recovery people called miraculous. Y/n never questioned it, not really. They just thought he got lucky.
Until that day.
The briefing room was quiet, the air stale with old coffee and fluorescent hum. Laswell stood near the table, arms crossed, a faint smile breaking the usual severity of her face.
"You know," she said casually, "if you never gave Soap your organ, he wouldn’t have been able to do the things he could now. Thank you, Y/n."
The words didn’t land right. They hit and kept echoing, long after the room fell silent.
Y/n froze. The pen in their hand slipped, clattering against the metal tabletop. Their pulse thundered in their ears.
"I never…" Their voice came out hoarse, thin, trembling at the edges. "I never gave my organ away…"
Laswell blinked, her expression faltering for the first time. "What?"
But Y/n wasn’t looking at her anymore. They were staring at nothing, trying to remember the paperwork, the white light, Price’s voice. Their stomach turned cold.
The room spun. Everything that once made sense, every "thank you," every look of pity, every quiet glance from the team. It all aligned in one horrific, silent truth. _ The common room smelled faintly of coffee and dust. The team was already there, sprawled across chairs and couches, casual for once. Their laughter and muted conversation froze when Y/n walked in.
The hallway lights reflected off their pale, tense face, eyes sharp and furious. Every step toward the table was measured, but the air seemed to shake around them.
"You butchered me without consent?" Y/n’s voice cracked, raw with anger and heartbreak, echoing off the walls. Their hands trembled. Not just with fury, but with years of pain, betrayal, and disbelief.
Price’s eyes darkened. He leaned forward, voice low but heavy with the weight of authority and guilt. "We had to make a decision."
"It wasn’t your decision to make." Y/n shouted, "You committed organ theft- medical battery!"
Ghost shifted in his seat, arms crossed, silent but tense. Soap flinched, looking guilty and small, he understood only part of it.
Y/n’s chest heaved, words almost failing them. "Years of infections, pain, fatigue… I thought I survived almost dying from the bullet. But no. It was you. You decided my life wasn’t important if his could be saved."
Gaz’s jaw tightened. "It wasn’t simple, Y/n. You saved him that day. We- everyone- made the call. We didn’t know if you’d make it either."
Price spoke to defend, "You signed-"
Y/n’s voice was bitter, cutting. "Signed? You made me sign while I was barely conscious! That’s not consent! That’s… that’s-" Their voice faltered, swallowed by the storm of emotion they’d held inside for years.
Price’s face softened slightly, though the sternness never left his voice. "We did what we had to do. You’re alive, Y/n. Soap’s alive. That’s what mattered."
Y/n stared at him, the weight of betrayal pressing down like a physical force. "What about me? What about my life? My pain? It left me partially disabled!"
"You’re not disabled…" Price protested, trying to damage control the shitty situation.
"I can't lift more than 20 pounds. I can't run like I used to. I can't go on missions like before."
Silence settled over the room, heavier than any battlefield ever had.
Y/n's organ, still lived inside of him, John "Soap" MacTravish. Soap’s eyes were downcast, guilt etched deep into his features.
"You stole the life I could have had."
Y/n’s voice dropped to a whisper, trembling with exhaustion and heartbreak. "I trusted you… all of you. And you took it. You took everything."
The room stayed quiet. No one moved. No one spoke. For the first time in years, the team felt the full weight of the consequences of their choice.

I hoped it hurt to read through it (Not my Grammer, lol) The feeling of betrayal about the people you make smut about and fluff stories.
This is my first (maybe only) angst story and there will be no part two.
Edit: there's a part two Here 🫡
