What is your name (first only) and what would you like us to call you?
What is your birthday and age? (depending on your age, there will be a few more questions that will be asked of you, if you are accepted.)
Do you have discord, facebook messenger, kik, or any other messenger app? Please list all you have. (I want to be able to have a group chat where we all could chat with one another, in case we need help or have ideas or just want to become friends ^-^)
Are you available to be active in the group at least 3 times a week? (I do not want people disappearing on me and leaving the blog empty. If you want to do this temporarily or if something happens to prevent you from being active, let me (VoltageShots) know and we will figure out what to do from there.)
Please list the Voltage men you are confident in role-playing as: (There is no maximum, list as many as you’d like. This does not necessarily mean that you will be all of them. Please include which game they are )
