Until he was an adult, Pantalone never let his hair grow out. He kept it short to make dealing with lice and matted or tangled hair easier. Cutting it away is easier and faster than trying to brush it out, and washing your hair can only do so much if you don't have anything to actually kill off the lice. The fact Pantalone is growing his hair out at all is an indicator of how far he's come, because now he can ACTUALLY take care of himself and only has to cut his hair if he WANTS to, not because he NEEDS to.
I think we can all agree that man has a thorough skincare routine but I have something more specific as both a headcanon and my own personal experience; he has tinea veriscolour. It's a type of non-contagious skin fungus. We all produce a bit of yeast in our skin, but if you have more than the average person, you may get these kind of scaley patches here and there, especially in spots where you sweat a lot. Pantalone in particular used to get it really bad around his neck and shoulders. It's not difficult to manage it so long as you have the right products, but hot and humid weather makes it worse, and in my experience you pretty much have to be on top of it the moment you see it or it'll take forever to get rid of.
He started wearing turtlenecks to hide his neck while he got his skin under control, then kept wearing them because they left weird tan spots on his skin, so it STILL looked like he had it for a while.
