Would do HRT for free...same with top and or bottom surgery heck if you wanted to be built like a doll no parts no nothing period man would do that shit for free he don't give a fuck about nothing he'd do it because he could he'd turn you into a robot if you wanted. Man is not a certified doctor soooo what do you expect?
To be honest he'd also do so even if you didn't want to if you were just one of his many test subjects but if you were a legitimate patient of his he'd do what you wanted no questions asked.
Dude could probably find a way to make irminsol change your freakin name and such so that all records would show your chosen name was always your name idk he'd be more than capable I think.

Would charge of course he's got a shop and bills to pay but he'd be pretty open about the whole process. He'd ask you questions what you'd like to do and tell you flat out what changes could or could not occur if you did HRT and what to expect and what to do.
Not sure how good he is at surgery I barely hear anything about surgery in Liyue so he may be good at it but he may not be good at it cosmetically.
Would advise you to take your time and be sure about every decision you make and perhaps even double check you are aware of certain changes that you may not be able to control such as weight distribution, hair loss, and etc.

He's more of a field medic than an actual doctor but he'd probably know a thing or two about the best surgeons and doctors to see in Sumeru if you wanted to do anything.
Would definitely help you figure out what exactly your goals are and what you can and cannot achieve with hormones and or surgery.
A pretty great listener who will let you know if your goals are realistic or too idealistic and help you figure out what is best for you in the end.
Man absolutely will scold you if you don't follow your doctor's instructions to the letter especially if surgery is involved.

More of a nurse than an actual doctor but more of a nun than either so the most she could do is help you figure out what you want to do and how to get it done and watch over you after surgery and or after you start HRT.
She's a bit confused but supportive and though she doesn't know much she's always willing to learn more and help out her friends.
She studies quite a bit about it so that when you do make a decision she knows what to do to help out like if you're the type to get nervous with needles but need to do injections she can help you do them or teach you better how to do them yourself.
Kokomi
Not a doctor, not a nurse, but a secret third thing...just kidding but she's not a medical professional by any means even if she can heal. I'd put her closer to a field medic like Tighnari.
Watatsumi is very isolated and they don't exactly have access to resources like the main island does so while they may be able to perform surgeries it could be dangerous and may not be cosmetically appealing.
She's still more than willing to listen to your troubles if she has enough energy to spare and see what she can do to get you what you need.
Any HRT stuff might be illegally acquired at least before the new trade laws existed.
Sigewinne
Is a nurse and pretty close to a doctor considering her skills. She's a great option to discuss this kind of stuff with especially since she isn't human and isn't bound by human conventions of what makes someone this or that.
She's listen very carefully and with her extensive knowledge can help you come up with a good plan to start your journey however you wish. Enjoys listening to how you want to change and why and all the little things too.
Will also scold you if you don't take care of yourself and do as your doctor and or surgeon asks.
Because of your changes she'll make you something that'll definitely help you heal and adjust to your newer self though there are no promises that it'll taste good.
