Posted @dr.lailarizvi Metal fillings can appear to be intact and still be leaking at the same time! 😩 Look at the decay and weak unsupported tooth structure after I removed the amalgam!! 😳 The premolar was restored with resin while the molar had surrounding walls and biting cusps that were less than 1 mm wide. So we know we have to remove all the ‘mushy’, decaying tooth and the old filling. 🤔 But how should we repair the hole left in the tooth? There are many dental materials available but the main decision is direct or indirect- resin vs porcelain. 🧐 Lets look at how much good, solid tooth we have left. How wide are the remaining walls? Have we lost any cusps of the tooth? How much of the biting surface has been compromised? 😘 After careful consideration, I decided to restore the tooth with an indirect, porcelain filling! After I knew he was comfortably numb, I removed the soft, decayed tooth structure and made a neat margin for the new crowns. I then scanned the prepared teeth, virtually designed core buildups (to rebuild the broken tooth structure) and began fabricating his new crowns out of E-Max porcelain. I then custom shaded his teeth and began ‘firing’ them right in the office! Meanwhile, I treated the teeth with calcium hydroxide to help preserve the pulp.
