For Chyrus the last time he hurt someone emotionally was Adonias. Honestly he really tried to be as nice as he could about the situation. But Adonias just was not ment for the Temple of Humility. Chyrus would not be representing his virtue if he wasn't trying to help Adonias be the best he could be.
Unfortunately Adonias didn't see it that way. He saw it as a failure. Chryus did get him transferred to another Temple.
For Adrestes it was Leonidus. I think trying to kill someone to the point they wet themselves a little begging for their life. Especially someone who looked up to you and had not done anything to you? It did cause Adrestes to snap out of his own paranoia. And he is deeply sorry for what he did to Leo.
He did also hurt Thenios but that wasn't his fault. He was under the Archons orders and Thenios did shoot the messenger when it came to Adrestes not allowing him to see Devos.

For Chyrus to put it nicely, sometimes he wants to call Adrestes a fucking asshole. He holds his tongue. As the Paragon of Humility he is supposed to help Kyrian learn their mistakes. But sometimes Adrestes is a bit too prideful of the Kyrian. Sometimes he looks down on people, including Chyrus. Chyrus isn't stupid but he knows Adrestes thinks he is. He knows Adrestes looks down on Humility until he needs it. Yes Aspirant Care is not the most lavish duty but you'd be complaining if there were no aspirants who could read.
For Adrestes, it's the fact that sometimes Chyrus takes jokes too far. For example Bong Dancer. Adrestes was terrified Chyrus didn't get how stigmatized tattoos are. And when Chyrus said it's fine Adrestes felt extremely brushed off. Chyrus loves being funny he loves trying to bring a chill vibe to things. But this can rub on Adrestes because 'this is serious it's not funny. '

For Chyrus it was Adonias. He didn't know that Devos was leading the Forsworn. He genuinely thought he was helping Adonias have a better life in Bastion. Adrestes even approved of Adonias becoming a Sentinel.
But Devos managed to doom Adonias. Helped amplify his ideas that Bastion wasn't fair and he was being punished for his personality.
Lysonia only encouraged Adonias more and when given the chance Adonias was pretty happy to torture Chyrus, with the intent as to death. This path ultimately lead to Adonias' own death.
Chyrus does partly blame himself for Adonias and also Fotima who was killed by Adonias. But he knows there's nothing he can do but honour their memories.
For Adrestes he really upsets Chryus in Lessons of Humility he tries to comfort him. But Adrestes does not have a medical background. He doesn't know how to drop harsh news gently. So he hits Chyrus with a double whammy of your arm got eaten and so did Merica. You're paragon now grow up about it. Which isn't what he ment but it came out that way. He was actually scared of Chyrus' for a bit because he'd never seen him loose his temper before. He was avoidant of Chyrus' until Chyrus confronted him.
