In today’s D&D session...

One of my players had reincarnate cast upon him via a Wild Magic surge (he would come back to life as if by said spell if he died within a minute), died like 10 seconds later and came back as a drow (was a half-elf). He then proceeded, via another two Wild Magic surges in two spells, polymorphed himself into a kuo-toa and then cast fireball on himself and nearly knocked out half the party.
