Cosmetic Correction of Anophthalmia Arising from Hemifacial Microsomia This little girl has hemifacial microsomia with microtia of the left ear along with anophthalmia of the left eye. Anophthalmia is the complete absence of the eye. She had undergone placement of a shell elsewhere, but the shell was very small and she still had asymmetrical eyes. Her parents brought her to our hospital for corrective surgery Treatment planning was explained to them and they consented to surgery. A subdermal fat graft was harvested from the gluteal region in order to augment the orbital cavity for shell placement. The gluteal region was chosen as the site of harvest in order to avoid any visible scar formation. Her palpebral fissure was also enlarged to increase eye volume and the fat graft was placed within the orbital cavity. The lateral canthal ligament was repositioned superiorly following which a new shell was fabricated and fitted into the orbit. There was very good cosmetic correction with symmetrical appearance of her eyes. She was very happy with the results of the surgery as the asymmetrical eyes had always distressed her.
