Le Fort III Facial Advancement for Crouzon Syndrome by Internal Distraction This young boy was born with Crouzon syndrome, which is manifested by premature fusion of skull bones (craniostenosis). The eyes are wide set and bulging due to shallow sockets. The middle face is also retruded. He had already undergone surgery for craniostenosis correction. Treatment planning was explained to his parents and they consented to surgery. A bicoronal flap was raised and the glabella and supraorbital notch were exposed. Cuts were then made in the zygomatic arch, frontozygomatic suture, floor of orbit and nasion. The vomer and ethmoid bone were disengaged from the cranial base. Pterygomaxillary dysjunction was performed and his entire midface was detached. Kawamoto internal distractors were then placed after dissection of the temporalis muscle. A total distraction of 18 mm was performed after a latency period of six days. The patient’s facial appearance normalized after the distraction was completed. Distractors were removed after consolidation of bone at distracted site in six months’ time. He also underwent a cosmetic rhinoplasty for crooked nose deformity.
