Le Fort I Osteotomy for Maxillary Advancement This teenager had been born with bilateral cleft lip and palate deformity and had undergone corrective surgery at 3 and 11 months respectively. She had later undergone bone graft placement to her alveolus at 7 years of age. She has an overjet of 10 mm and now presents for correction of a hypoplastic maxilla. Treatment planning was explained to her and her parents and they consented to surgery. A Le Fort I osteotomy was performed and the maxilla was advanced by 12 mm. This resulted in correction of her overjet. Occlusion was checked and the maxilla was stabilized with plates and screws. Le Fort III Facial Advancement for Crouzon Syndrome by Internal Distraction This young boy was born with Crouzon syndrome, which is manifested by premature fusion of skull bones (craniostenosis). The eyes are wide set and bulging due to shallow sockets. The middle face is also retruded. He had already undergone surgery for craniostenosis correction. Treatment planning was explained to his parents and they consented to surgery. A bicoronal flap was raised and the glabella and supraorbital notch were exposed. Cuts were then made in the zygomatic arch, frontozygomatic suture, floor of orbit and nasion. The vomer and ethmoid bone were disengaged from the cranial base. Pterygomaxillary dysjunction was performed and his entire midface was detached. Kawamoto internal distractors were then placed after dissection of the temporalis muscle. A total distraction of 18 mm was performed after a latency period of six days. The patient’s facial appearance normalized after the distraction was completed. Distractors were removed after consolidation of bone at distracted site in six months’ time. He also underwent a cosmetic rhinoplasty for crooked nose deformity. He presented to our hospital for corrective surgery. A 3D CT obtained at our hospital revealed a decreased ramus height on the left side along with an occlusal cant. Le Fort I Osteotomy with Maxillary Advancement This adolescent girl had been born with a unilateral cleft lip and palate. She undergone cleft lip and palate surgery as an infant followed by bone grafting at 7 years of age. She now presents with a retruded midface and an anterior crossbite of 6 mm, which is very common for adolescents with this deformity. Treatment planning was explained to her and she consented to surgery. She underwent orthodontic alignment before surgery. A Le Fort I maxillary advancement of 8 mm was performed thus correcting her retruded midfacial defect along with the anterior crossbite. Unilateral Cleft Lip Repair This little girl was born with a unilateral cleft lip and palate deformity. The ideal time for surgical repair of a cleft lip is at around 3 months old. Her parents however could not bring her at that time and presented with her when she was 7 months old. Dr SM Balaji explained the treatment plan to them in detail and they consented to surgery. A C-flap was raised to reconstruct the floor of the nose and the nasal sill. Vermillion border was then reconstructed with a three layer closure of the cleft deformity with perfect approximation of the orbicularis oris, subcutaneous tissue and skin. She now presents with a retruded midface due to maxillary hypoplasia. This has resulted in an anterior crossbite. She also has a missing maxillary right lateral incisor. Treatment planning was explained to her and she consented to surgery. She first underwent fixed orthodontic treatment to decompensate her maxillary arch. A dental implant was then fixed to replace her missing lateral incisor followed by a ceramic crown after osseointegration of the implant to surrounding alveolar bone.
