Cas and Dean get separated in a night club during a case. Dean discovers the witch dosed Cas with a love potion so when he spots Cas passed out in the alley down below he rushes to him. Dean busts through the back door the same time Sam calls to him - and Dean freaks out: Sam is knelt beside a groaning Cas. Dean yells at Cas not to open his eyes as he runs to his side, skidding to a stop and out of breath. He forces a palm over Cas' eyes just in case; the spell set to be 'activated' by the first person Cas sees - and Dean sure as hell isn't gonna let Cas go from mooney-eyes to dangerously-devoted to anyone, least of all Sammy. But then Cas is convulsing violently and they move to stabilise him - Dean loses his grip over Cas' eyes. After a minute, Cas calms. Dean braces his face gently with two hands; Cas is too still. Maybe the potion is poisonous to Angels? Dean's stomach drops out and he wills Cas to open his eyes - begs it out loud. Cas stirs as tears well and Dean forgets caution as Cas' eyes focus on him; it's a good feeling. Relief. Joy. Then comes the terror as the reality of their situation returns and Cas squints, still fixated on Dean. No, Dean thinks, I won't let him go crazy over me, the potion meant to eventually make Cas sacrifice himself for Dean's sake.

I think it's fair to say the Mark of Cain would affect an Angel differently so when vision!Dean says the mark made Cas "go crazy" he means it because whereas moc!Dean was overcome by near insatiable bloodlust moc!Cas has Angelic Grace which doesn't mix well with the curse so the result isn't an unstoppable killing machine but rather an unhinged dangerous powerhouse basically the Angelic equivalent of a feral animal driven by instinct with no chance of reasoning with him because his sanity has been corrupted
