JP finally explained! When Silas sucked it out of Katherine, it reverted her back to her normal age- aka 500+ years old. She said Elena could survive the cure being taken bc she's only biologically in her 20s. But Damon would die like Katherine.

Also... if this is true, shouldn’t they be offering it to other new vampires before Damon takes it? lol
