I think it's more like Sherlock knew that this is what Moriaty was doing. So when he was talking to his friend, the doctor. (the female that had a thing for him) he was telling her what was going to happen and he would need her medical skills to make him survive. Then that motorcycle guy that crashed into John was also planned to let Sherlock have enough time to make it seem like his heart beat was gone (some criminal did that in some other episode) So when they took the body inside, the doctor just did whatever she had to do and she + the mysterious driver are the only ones to know that Sherlock is still alive.
