Dean offers for cas to come back with them to the bunker at the end of 11x10, right after they all got out of hell.
The last time Dean was in hell was when he was tortured for 30 years until he gave in to torturing others. Last time, Cas saved him. Last time, Dean had nightmares and couldn’t admit to even remembering what happened there. Right here, Dean is saying “I need you with me,” and because Dean has no idea cas is possessed by Lucifer here, it absolutely wrecks him that cas chooses to leave when his wounds have just been opened again.
