Despite the fact that both Death Tests have only one level as I recall, and overlooking the unfortunate choice to print the box cover in preppy pink and green, I like the idea of the access hole in the floor/ceiling instead of the usual stairs. D&D module B1 also includes a room with a hole that can be used to climb down if the players figure out how to fasten a rope. I often add a similar access hole in my own dungeon maps, inspired by those examples.
